I, the undersigned hereby make application as an independent contractor umpire for the Southern Collegiate Umpires Association, Inc. and agree to comply will all policies and by-laws of SCUA. My permission is hereby granted for SCUA, Inc., to conduct a background and credit check at any time. If accepted, I understand and accept that my status as an independent contractor is for the current year only. Furthermore, I agree to release and hold harmless SCUA from any liability for any actions, such as, but not limited to: federal / state taxes, insurance / workers compensation
